your prescription medications via this new program.

Your Income is Under 100% of Poverty
During 2005 100% is:
Individuals - $ 9,570
Couples - $ 12,8830
If you are under those levels then:
You may sign up for free(no monthly premiums or annual deductibles)
and pay the following co-pays:
$ 1.00 for generic meds
$ 3.00 for brand name meds
Nothing after $ 5,100 in total annual drug costs
Or:
.Your Income is under 135 % of Poverty
During 2005 135 % is:
Individuals - $ 12,919
Couples - $ 12,320
If you are under those levels then:
You may sign up for free(no monthly premiums or annual
deductibles) and pay the following co-pays:
$ 2.00 for generic meds
$ 5.00 for brand name meds
Nothing after $ 5,100 in total annual drug costs
2. You Are Not on Medicaid (But you are on
Medicare)
Your income is under 135 % of poverty(see
guidelines above)
And your assets are below:
$ 7,500 for individuals
$ 12,000 for couples
You may sign up for free(no monthly premiums or annual
deductibles) and pay the following co-pays:
$ 2.00 for generic brands
$ 5.00 for brand name meds
Nothing after $ 5,100 in total annual drug costs.
Or:
Your income is under 150% of poverty
During 2005 150% is:
$ 14,355 for individuals
$ 19,245 for couples
And your assets are below:
$ 11,500 for individuals
$ 23,000 for couples
You may sign up for a:
Monthly premium based on a sliding fee from $ 0 to $ 28.00.
Deductible of $ 50.00
Co-pays of 15 %
After $ 5,100 in total annual drug costs then $ 2.00 for generic and
$ 5.00 for brand name meds.
